SMACK-417 If both PacketReader and PacketWriter fail at the same time, connectionClosedonError() is called two times
Refactored notifyConnectionError() and notifyReconnection() from PacketReader to XMPPConnection. Made PacketReader.done and PacketWriter.done volatile.
